That's called the Power Base Converter. It allows you to play Master System games on your Genesis. Great idea but it doesn't fit on the Genesis 2 (that's the model he has in the pic) unless you cut out the back part of it since it was designed for the original Genesis model (the one with some red on it). The Genesis 2 was smaller but wider (if you haven't seen the Genesis 3, it's TINY, noticeably smaller than an original Xbox controller). Seems like the only way he's even able to get it on that Genesis is to use the 32X in between.
Although the cleaning system is insanely tall and he's using two Sonic & Knuckles carts, it's still pretty insane. Woulda been even taller if he used the Sega CD 1 instead of the 2. That thing's a bit taller than the second model
